LTMGH Blood bank is a vital service department of LTMG
Hospital. LTMGH blood bank functions under supervision of Department of
Pathology of LTMG hospital. It works 24*7 to give services to needy and poor
patients. It is one of the most important emergency service departments which
also plays important role in disaster management.
Blood transfusion is a life saving measure in cases of
massive blood loss, major surgeries, obstetrics and gynecology patients,
chronic anemia, renal hemodialysis patients, cardiac surgeries, aplastic anemia, leukemia undergoing
chemotherapy and also in conditions which require multiple blood transfusion like thalassemia, sickle cell
anemia etc.

Importance of Voluntary Blood Donation :
Blood is a precious product from human source. Blood donation
is one’s commitment towards society, also gives a feeling of immense satisfaction
to the donor. Voluntary blood donation is the safest way of providing blood of
assured quality.
The donated blood unit is separated into components: Red Cell
Concentrate, Platelet Concentrate, Fresh Frozen Plasma (FFP) and
cryoprecipitate. These blood components are issued to 4 different patients as
per the clinical needs. Thus, a single unit of blood can save lives of FOUR
patients.

Plateletpheresis (Single
Donor Platelets):
“Plateletpheresis” is a special procedure by which
only the platelets are separated from the donor and the rest of the components
i.e. red cells and plasma are infused back into the donor.
A single platelet donation (SDP) can provide platelets almost
five times one can obtain through random donor platelets (RDP).
These platelets are lifesaving for patients who suffer from
bleeding due to monsoon related illnesses like malaria, leptospirosis and
dengue. They are therapeutically very effective for patients with blood cancers
and aplastic anemia who have low platelets. Patients undergoing heart surgery
also benefit from these platelet products.
Platelet
donation facility is available in the LTMGH Blood bank for the benefit of
patients who are admitted in LTMG Hospital. The blood
bank has well equipped apheresis machine which are used for the purpose of
plateletpheresis. These
SDPs are given “free of cost”.
Being a tertiary care public hospital, with largest trauma Centre
of Mumbai city and multiple superspeciality departments, patients from all over
Mumbai come to LTMG Hospital for treatment of various diseases, many of which
require blood. The requirement of blood and blood components is about 80-100
units per day at LTMG. Hospital.

Collection of blood from donors:
The
Department collects blood from voluntary blood donors. More than 130-150 blood
donation camps are conducted annually with various voluntary organizations,
colleges, corporate offices and many more. Besides this, blood is also
collected from blood donors who come to the blood bank for donation. The annual
blood collection at the blood bank is 16,000- 18,000 units
§ Providing
properly grouped and crossmatched blood
All blood units are processed for cell and serum grouping. Serum grouping
confirms findings of cell grouping and also detects rare atypical antibodies.
Confirmatory testing (Du test) is done on all units which test as Rh negative
to rule out weak or partial Rh groups.
§ Detection
of transfusion transmitted infections – HIV, Hepatitis B, Hepatitis
C, Syphilis and Malaria
Each and every blood unit collected is tested. Detection of anti-HIV-1 & -2
and Hepatitis is done by sensitive ELISA kits using automated washers and computerized
ELISA readers.
§ Ensuring
optimal utilization of blood by preparing blood components
Almost all the blood collected is separated into the various blood components.
The Blood Bank has special refrigerated cryo- centrifuges and plasma expressors
as well as experienced staff for the separation of these components.
§ Proper
preservation of blood
All blood units are preserved at 2-6 ° C, in a well-designed
walk-in cooler equipped with a continuous temperature monitor and alarm
facility. Plasma and Cryo-precipitate are preserved in large freezers
maintaining temperature below -30° C, while platelet concentrates are kept on a
special rotator for continuous slow agitation which is essential for their
proper functioning.
§ Prompt
and appropriate disposal of unsafe blood
All units of blood suspected to harbor infectious agents or
be sub-optimal for any reason are carefully autoclaved at the Blood Bank and
destroyed by incineration at the Municipal incinerator.
§ Promptly
provide blood, round the clock
The
Blood Bank crossmatches and issues blood and components continuously, all the
twenty-four hours, thus making it available during all the emergency hours.
Under
the supervision of the Faculty, technical supervisors, technicians, medical
social workers, staff nurses, clerical staff and support staff are working in
various sections of the department.
The Blood Bank Department includes :

Blood Donation Centre: The
department has a blood and Apheresis donation Room which collects blood from
100% voluntary non-remunerated donors. The Centre also provides pre and post
donation counselling services with the help of trained counsellors. The blood
donation service is available from 9.00am to 4.00 pm on all weekdays and 9.00
am to 1.00 pm on Saturdays.

Red cell serology Lab: is
a testing laboratory and performs ABO & Rh blood grouping, Cross matching,
Screening & Identification of antibodies, to resolve red cell
discrepancies. This technology has helped to reduce turnaround time of blood
issue significantly. The team of trained doctors and technologists provide
fast, reliable and accurate results to save patient lives.

Component Lab: The
donated blood is separated into its various components in a well-equipped lab with
advanced technology and automation. This ensures a regular supply of safe blood
components to meet various patient requirements.

TTI Testing Lab: The
lab for transfusion transmitted infectious disease testing of donated blood,
that screen blood for antibodies to HIV1 & 2, Hepatitis C virus and
Hepatitis B surface antigen. Additional tests are performed to screen blood for
Syphilis & Malarial parasite using high quality and sensitive antigen
methods.

Component Storage Lab: The
lab is well equipped with Freezers and Refrigerators that are capable of
storing blood and its components at ideal temperatures 24x7 and thus ensures
the quality and reliability of components stored.

clock to ensure the quality and safety of blood and blood products to all
patients.
What is
the process for Blood Donation?

1stStep: Registration
Donor are provided with a Questionnaire to ensure that he/she is fit and his/her
blood is safe for a patient. The information provided will be kept
confidential. The form should be filled correctly and honestly.
·
2nd Step : Medical
screening
Donor Hemoglobin will be checked by a finger prick method to ensure that he/she
are not anemic. The blood pressure will be checked prior to donation. Questions
will be asked to ensure your blood is safe for patient. If donor is disqualified, he/she will be
explained why the same and given appropriate medical advice.

3rd Step : Counseling
Pre donation and post donation counselling is done by expert Counsellors asking certain questions regarding health in
confidential manner. If donors turned out to be positive for Transfusion
transmitted diseases, then they are taken to respective section and treatment
is provided free of cost.

4th Step: Phlebotomy (Blood Donation Procedure)
Donor will be made to lie down on donor couch. The skin of the inner elbow area
will be cleaned antiseptically. An experienced phlebotomist will insert a
sterile needle into vein. The donation will take just 5-10 mins. After donation,
donor will be asked to lie down for another 5 mins. with the punctured arm
folded at the elbow.

5th Step : Refreshment
Donor will be served refreshments after
donation. A Donor card & Certificate will be handed over. Donor will be guided regarding post donation
care. A sticking plaster will be applied at the punctured area.

Who can donate blood and platelets?
·
If
you are between 18 and 65 years of age, weighing more than 45 kg and are
healthy, you can donate blood. For platelet donation, the minimum weight should
be 55 kg. You should have eaten a non-oily healthy breakfast/ meal within 4
hours before coming for blood donation. Additionally, the doctors in the blood
bank will explain to you the benefits of as well as the eligibility criteria for
blood donation.

Why is voluntary blood
donation encouraged?
·
In
the days of blood communicable infections, repeat voluntary donors who donate
without any form of remuneration, are the cornerstone of a safe and adequate
blood supply.

Why is 3 months interval
required between two donations ?
·
Although
the lost blood is replenished within 6 weeks, an additional 6 weeks is required
for the safety of the donor. For SDP donation, one can donate platelets at
interval of 72 hours.
·
Does one feel weak after
donating blood ?
·
Blood
donation does not have any adverse effects on the body and one can go back to
work soon after donation.

What Is a Platelet Apheresis
Donation or SDP Donation?
·
During
a platelet donation, a small portion of your blood is drawn from your arm and
passed through an apheresis machine. The machine collects the platelets and
safely returns the remaining blood components, back. After the donation you can
resume your normal activities.
